Warning: Spoilers ahead for the Scrubs revival’s double-episode premiere!Scrubs surprised everyone when the revival replaced John C. McGinley’s Dr. Cox with Zach Braff’s JD as Sacred Heart’s new Chief of Medicine. It was a twist that was kept under wraps incredibly well. While the move makes sense given how times have changed since the show ended more than 15 years ago, the transition from Dr. Cox to JD arguably comes with a flaw in its logic. However, said flaw doesn’t actually exist. The reason why Dr. Cox gave the job to JD over anyone else is perfectly clear.
After taking over as acting Chief of Medicine in Scrubs Season 7, Dr. Cox officially inherited the role in Season 8. Before the revival’s premiere, it was largely expected that he would retain the title as Sacred Heart’s leader for a substantially longer period of time. While it made storytelling sense for JD to succeed his mentor eventually, it happened far sooner than anyone expected. Still, if Cox had been thinking about hanging up his stethoscope before JD’s return, there’s only really one other doctor who would have been up to the task.
Why Dr. Cox Didn’t Choose Elliot as His Successor at Sacred Heart
Elliot (Sarah Chalke) started at Sacred Heart at exactly the same time as JD did back in the 2001 pilot. Since then, other than an incredibly brief stint at another hospital in Scrubs Season 5, Elliot has been at Sacred Heart for her entire career, and rivals JD in her professional commitment and vast medical knowledge. So, with Dr. Cox clearly looking for someone to replace him, it would be forgivable to wonder why he never entertained the idea of asking Elliot to take over as Chief of Medicine. She would certainly be capable of filling the role.
Then, when Elliot’s career is considered, it becomes clear. Toward the end of Scrubs‘ original run, Elliot stops being an employee of Sacred Heart. While she continues to operate out of the same hospital as her friends and colleagues, she actually starts her own private practice. Therefore, it would be logistically challenging, or even impossible, for Elliot to continue running her practice while also being responsible for the day-to-day management of Sacred Heart. Furthermore, practicing medicine privately can often be more financially lucrative, even when compared to being a Chief of Medicine. So, Dr. Cox likely didn’t think it was worthwhile asking Elliot if she was interested in the job.
There’s Another Reason Why Dr. Cox Chose JD Over Elliot
Even ignoring the fact that Elliot would essentially need to abandon her private practice in order to become Sacred Heart’s new Chief, I heavily doubt that Dr. Cox ever even entertained the idea. His relationship with Elliot has always been fractious, almost to the point of them hating each other. Their differing ways of doing things have resulted in them butting heads on countless occasions. While they have their moments of professional courtesy, that’s pretty much it. There is an argument to suggest this shouldn’t matter, as Dr. Cox hired Turk (Donald Faison) as Chief of Surgery in Season 8, and the two men had also never really seen eye-to-eye. Dr. Cox was able to see past this, though, and ultimately gave Turk the job because he was the most qualified.
For Dr. Cox to have ever considered Elliot for Chief of Medicine, she probably would have needed not to break up with JD and ask for a divorce. Although he has a history of hiding how he feels about JD, Dr. Cox sees Braff’s character as his surrogate son and is fiercely protective when it comes to his well-being. The latest sign of this trait is when Dr. Cox slates Elliot for claiming how hard it is on her that JD has come back to Sacred Heart, offering the sarcastic sentiment of: “Ah, yes. In the history of relationships, it has always been hardest on the one who ends it.” Pair this deep dislike of what Elliot did to JD with the fact that Dr. Cox imbued many of his values into his successor, and it makes even more sense why the Scrubs revival passed over Elliot for the new Chief of Medicine.
