As you explore Act 3 of Path of Exile 2, you may struggle to use the Sacrificial Heart, one item you may pick up from enemies you encounter. This quest item is described as “a soul still clinging to fading shreds of life,” offering you an eternal boon for your character. However, you can only sacrifice the heart “at the proper ritual site,” which can be unclear at first.
Before you can get the Sacrificial Heart, you must first progress through enough of the game to reach Act 3. This stage of the main story takes place when you use the portal in the Ziggurat Encampment to travel back in time. The areas you explore and the NPCs you meet in Path of Exile 2 here will be new, including the loot you receive from enemies.

Defeat The Right Enemies In The Right Areas
The Sacrificial Heart will drop from enemies in the following areas:
Utzaal is the first area you explore after using the Ziggurat portal, so you may pick up the Heart only after a few enemy encounters. However, others may have to defeat multiple hordes before finding the item they seek. Don’t be afraid to die and reset areas where the Sacrificial Heart might be found to reset enemy spawns and increase the chance of the item dropping.
Farming different enemies while looking for the Sacrificial Heart is a great way to farm other materials and loot needed for crafting gear in Path of Exile 2 or leveling up your character.
Some enemies, like Vaal Goliaths and Vaal Overseers, are the most likely sources to drop a Sacrificial Heart in Utzaal. Aggorat also hosts these enemies, although it is a region much harder to access than Utzaal from the portal. An easy way to reach Aggorat is to defeat the Legion Blockade boss in Utzaal, where you can hear chanting, then go behind the giant statue in the area and up the stairs leading to the new location.

If you are having trouble finding the Sacrificial Heart from either location, finding checkpoints to help reset enemiesis a good idea. The more checkpoints you have in Utzaal or Aggorat, the more places you can look or respawn at. You may pick up the Sacrificial Heart by accident as you wipe out enemies, so try to use a loot filter in Path of Exile 2 to check your inventory to see if you picked one up already.
How To Use The Sacrificial Heart
Find The Right Altar In Aggorat
Once you have a Sacrificial Heart, return to Aggorat and look for the Blood Sacrifice along the Trail of Sacrifice in this area. This place is marked with a checkpoint, overseeing a multitude of dead bodies bleeding into the stones. Stairs here lead up to a massive altar looking over the civilization below, with an item called the Sacrificial Dagger ready to be used alongside the Heart.

Pick up the Sacrificial Dagger and place the Sacrificial Heart at the center of the Altar to begin a blood ritual. When you place the Heart down with the Dagger in your inventory, you will be given an option to “Stab Sacrificial Heart.” Perform this task to play a short cutscene where your character kneels and stabs the Heart, removing it from your inventory.
Destroying the Sacrificial Heart grants you a permanent stat bonus in Path of Exile 2, granting your character:
- +2 Passive Skill Points
- +2 Weapon Skill Points
These skill points can be spent on whatever abilities you wish, allowing you to refine your character build further as you grow stronger. Nothing can take these points away, so you have infinite freedom to use them in different ways. For example, you could invest in passive skills tied to damage reduction against certain elements for extra survivability in later Acts of the game.
You can collect another Sacrificial Heart on Cruel difficulties and destroy it again at the same Aggorat altar. Doing this on Cruel difficulty doesn’t change the permanent bonus, but it gives you a second instance if you’ve already performed the sacrifice in the base game.
There is little reason not to collect these skill points, so you should always try to do this task on every playthrough. Learning to use the Sacrificial Heart in Path of Exile 2 gives you a free set of skill points you don’t need to grind for, making the effort of finding the Heart worth it.
